I do everything by appointment and under written contract, and operate on a retainer system, similar to what many attorneys use. My standard billable rate is $150/hr, but I prefer to work and bill on a more cost-efficient, per-diem basis. There is no charge for a brief (up to 1 hour) consultation, and most contracts can be fulfilled at whatever pace your budget will allow.
